Problem:
poorly debugged DX11 code, resulting in general DXGI_Error_Device_Removed return message.
Fix:
C:\Program Files (x86)\Steam\SteamApps\common\Star Trek Online\localdata\GamePrefs.pref
- Open in notepad
- Set Prefentry SystemSpecs.Isdx11enabled to value 0
- Make the file read only.
If game still hangs:
C:\Program Files (x86)\Steam\SteamApps\common\Star Trek Online\Star Trek Online\Live\Localdata\GamePrefs.pref
- Open in notepad
- Set PrefEntry GfxSettings.Antialiasing to value 0
- Set PrefEntry GfxSettings.Fullscreen to value 0
- Make the file read only.
this was very helpful thnk your hope this will be add to the next patch and we need to take this files away before the patch am i right because i think read only could cause problems for the patch ?!
Great advise. question. When you save the file. What do you save it as?
I haven't had to use this, but assuming you're editing a file in notepad, save it as exactly the same file and overwrite, so in this case, GamePrefs.pref then make it read-only as OP suggested.

I have followed the first part of your post, with no resulting change. I attempted to follow the second section but cannot locate any items that match your listings.

Okay not a steam user but found the file in the perfect world folder. But it gives me access denied when I try and save.

Sorry, but I don't find this particularly good advice. For one, there's nothing wrong with DX11 in this game (have you ever seen those Haywire plasma bolst glistening off your ship's bow?! In DX11 you can!). Maybe you just need a better card?! Newer drivers?
Secondly, if you're going to disable DX11 and/or Anti-Aliasing, just use the ingame settings menu to lower your graphics settings.

There have been many players, myself included, which had problems with DX11 after S10. It will crash STO while in loadingscreen (the one when you start the game), giving an DX11-Error, even if you dont have a DX11-card and DX11 installed (its hard to use something thats not there I presume). Apparently the game wants it so badly, it doesnt make an exception.
For me it helped to give the commandline -d3d9 to the launcher, thus forcing the game to start with DX9.
That didnt happen prior to S10, not even on the same day
Though the game still crashes firefox after entering the character selection for the first time

Strange. I have an nVidia GTX 980 (GTX 580 before that), and DX11 *never* makes my system crash; like ever. Maybe Cryptic really don't like AMD?!
If the file is in program files you probably needed to run Notepad as administrator to be able to save changes.
The other possibility is you need to reboot and make sure no STO processes are running that have locked the file.
You might need to do both.
Do you have an AMD Radeon graphics card? The STO client seems to prefer nvidia.
NVIDIA GeForce GT520
And I did what a later poster said and saved it out of the folder then it let me drop it into the folder with admin privileges. (Oddly it did not let me open with admin.)
Doing this got me into Neverwinter last night with no graphics lost. And I can see why it is needed since the system keeps reverting changes I make in game. (IE I have to keep saying to play in full screen not a minimized window.)

The error message actually says it all. The 3D engine on your GFX card is electrically disconnecting itself from the PCI bus. One reason might be that when the card switches to DX11 mode it consumes more power. If your PSU can't keep up, the voltage will drop and the electronics on that part of the GFX card will shut down. This can happen with any part in your computer but the GFX card is especially power hungry and will fail first.

No...
The 'device removed' error CAN mean the card has fallen out of the slot, or switched off, but usually doesn't.
That error is a bit of a catch-all, depending on what the return code is. The list of return codes that error message uses can include drivers updating while the app is running, screen resolution changes, and the app sending garbage directx api calls.
Evidence from the reports on the forums, of when and where, people have problems, and how they fared trying the various workarounds, strongly suggests the bad directx-11 api call from the game, not a wide assortment of cards from several makers all suddenly powering down because "maybe directx-11 uses more electricity".

This really helped! It turns out the problem for me was that I had had the graphics turned up really high before the game updated, and after it updated it was too complicated for my computer to handle. Thanks for ending a three day problem-solving nightmare!

That's nice ..... but, what if, and it's a BIG if, you can't get INTO the game to change those settings? ... Oh, wait ...

Version 2 crashed in the usual spot - right as the Iconian War splashscreen loaded.
