Fleet Tactical Console Quality Improvement Resets at max level

Problem with Mark XIV Fleet Tactical Console at Ultra Rare and next upgrade is quality to Epic/Legendary not keeping quality chance as intended.
Tooltip states:
If the item is at Maximum Mark, further upgrades can only improve quality, the chance is not reset, and quality improvement chance will continue to increase with each upgrade attempt.
Issue:
My maximum mark Fleet Tactical Console when upgraded reset to 0% quality chance instead of retaining its quality improvement chance as per the above in game tooltip.
Console:

Previously known as when originally purchased (i.e. before upgrading):
[Console - Tactical - Vulnerability Locator Mk XII [+Pha]
