Sounds similar to what has been happening to me.
I'm seeing the wrong bridge officer in the pop up window for various things throughout the game, such as returning to sector space while inside my fleet dilithium mine. I get a window with my boff's face asking if I'm sure I want to leave, but the name under the boff's head is that of 1 of my other boffs. It's happening all over the game it seems.
I'm not seeing generic boff's heads. I just have the wrong name under the boff's head, or the wrong boff showing in the window. So I'm seeing names and faces of my actual boffs. It could be related to your problem.
this has been a "feature" of Starfleet academy for as long as SFA has been in the game. If you hail you boffs to beam up it always shows the name of your science officer, but it shows the face of someone else.... most of the time. the officer shown varies, and will occasionally be your sci boff.
