wrong disc/modifcations/icon on Singularity warp core

just completed the Doff assignment and got this singularity warp core and the name, description, and icon don't match. has this happen to anyone else?

    It's happened several times with different items in the past. The icon's item, rarity, and mk # will be different from what is stated in the name, which is also different from the item's stated stats in the description.
