Adapted Honor Guard Helmet Missing

I still have just the two helmets for Honor Guard after completing the Adapted costume mission. I have all of my optionals, Elite n Normal. I have the other pieces for the Quv set... Shoulders, arms, wrists, body, chest, belt, boots... but only the Standard and the Hos Helmets.

I seem to remember before the STFs rewards changed, after getting the 6 Elite optionals, I already had 3 helmets.

Also... why does it say "Costume 4 Unlock" in the Rep Store when its really only 3??
