Would not be so radical like you saying, but I agree that space is much more bigger then the alpha quadrant.... The wormwhole in DS9 would be a great way of adding a new map to the game with more species, missions and the threat of the Dominion.

I would implement a nemesis system. Think of CO but obviously tweaked to fit STO. This would introduce a "intruder alert" scenario where you obviously have to repel your nemesis' crew off your ship.
Well were not in charge so i dont realy see the point. But IF i was, i would remove all borders in the galaxy map. Including the ones between known sector blocks and exploration clusters.
I would fix the randomization of said exploration clusters to include or exclude certain scenario's based on the random race chosen. I would make interiors of bases and ships make more sense. Not the mazelike structure we get now. I would add "optional" side missions that dont show up untill you run around and scan plantlife/wildlife. Or run around on a mountainy map to discover caves with crystals and whatever... So that people who realy love to explore can spend more time on a map. And others who dont just do the main mish and beam out.
I would make player ships stop exploding and start an interior based mission when a ship is low hp (5% hull) with different objectives inside based on class. Engineer would repair battle damage. Tac would fend off boarders. Science would heal injured in red alert lit hallways. And at the end you would go back to your bridge and choose to either restart the fight where you "died" or rather got disabled OR do a short warp jump so you end up on the edge of the map of the mission you were doing so you can try a different approach to the battle you lost before.
I would add a ground set that allows you to convert exploit to expose, or expose to exploit. (set bonus)
I would remove the gold looks of the new Doffs assignments, and Remove some of the slots that force you to take a doff based on traits instead of their specialty.
I would add missions that only allow you to bring 1-2 Boffs instead of the usual 4. And i would fix it so that you can always bring at least one.

Besides, with the war going on between the Federation, Klingons, Borg, Romulans, Cardassians, Borg, Species 8472, and now the Tholians, I don't think exploration is the Federation's top priority, and therefore not something you would likely see soon (or at least until the next season).

Do remember that even though you;re in charge of STO, you;re still answerable to PeeWee. And that they're the only shareholders at they have 100% of the Cryptic stock.
1. Complete the Klingon faction and have more costume items for sale in the C-Store.
2. Romulan faction.
3. Change space completely, make it somewhat closer to Eve's, but no jump gates as you'd be limited by your war drive capabilities. It would be somewhat more "sandbox" within the confines of Starfleet/KDF etc.
4. Non-Faction 'groups' where you can forge your own destiny, whether its through trade (ala Eve) or mercenary work it would be up to you.
Probably too radical, so I'd settle for 1 and 2 for now as the most critical. KDF faction needs more content to attract and retain more players. The academy set is 'meh', I am not interested too much in it, however if they made the mercenary set cross faction I would get that. I think they will get there...
After this its just making sure they continue as they pretty much have been.

And if someone likes that game mechanic and wants it to be in STO that does not mean they want the game to be like LOTRO. It actually fits perfectly with factions like the Borg or Undine.
I would fix the numerous bugs with this game. After this, I would turn my attentions to adding more episodes for both the Federation and Klingon factions. As this game is F2P, I can't see a way to remove lockboxes or lower prices in the c-store as these are big money earners for the game. Without them, the whole operation would be broke pretty quickly.
I would add rewards for the best foundry missions (XP, level specific items) and add them to a relevant chapter in game. The foundry would be an excellent tool for adding game content without having a large overhead that featured series have.
Ideally, if PWE had endless amount of cash to spare, I would expand on the exploration idea posted earlier, as well revamp the PvP system so more people want to take part in that section of the game, and introduce more playable factions with the same level of content as the Federation.
I think if I were in charge, I focus on a couple of things.
1. No new systems. Instead, polish and integrate what is already in the game. For example, integrate starbases with the Foundry, whether that means playing ugc in the holodeck or being able to use the Foundry to build social zones attached to doors in the starbases. Or, put crafting in the starbase and get rid of MA.
Other examples should be obvious. There are so many elements of this game and its existing systems that are just sloppy and never retouched. From dualing on Andoria to bugs to "You have earned 17 tokens" in Colony Invasion. It's like once a season goes live, the devs don't really care to fix a typo of the past, let alone be concerned that something is working as intended (like the "new" listings for Foundry searching that is forever borked due to auto-republishing).
There are some huge bugs that have big effects. But, if they aren't part of something new, or if they aren't attached to a store, they get ignored.
Basically, work with what we have, add polish, consistency, standardized ways of quest grants, turning in rewards, etc. "Is this working as intended?" should be the guiding question.
Show us that you are indeed embarrassed about certain things and that you care to fix them, instead of moving along to some new system that will add further inconsistencies to game as a whole.
2. Put story back into the game. Much of the plot of this game has been stripped. Entire parts of the path are forever unresolved. If Cryptic is incapable of producing story content, resolving its cliff hangers, and even giving a back story to the Klingon war, then pass the torch to Foundry authors who can. Reward players for playing story, instead of mindlessly mashing buttons to get some shiny.
3. Delete what is bad. Star Clusters with terrible missions. "Explore Unknown World" followed by "Starfleet, where have you been?" Delete the popups on Andoria if you're not going to add duals. If no one is playing the Crystalline Entity, and if you don't have plans or an inclination to improve it, then delete it from the game. It's a big red pimple, and no one likes to look at it.
4. Improve in game instructions, tutorials, and directions. If you have to go to a forum or website to understand a system, where to go, or how to do something, then the game has failed in properly teaching a player how to play. "Cryptic" shouldn't be a design philosophy.
Do something with SFA. Make it a central hub where players go to learn about the game, beyond just doffs and some lore. Make training a requirement. If I'm confused about something, I should be able to learn about in the game, and there is a spot to learn stuff in STO. Everything from kits to quests to systems... there should be a course at the academy.

I'm not sure what, but i would do something with Transwarp Gates.
There has to be more to this awesome Technology than what STO shows us.

Disable a ship, ready boarding parties in EV suits (in case of hostile environments or attempts by the enemy to flush you out into space), then a series of ground missions to seize key points on the enemy ship (possibly with some Doff style micromanagement involved as other squads tackle different objectives).
These missions would be very difficult (varying depending on the size of the ship and its crew), with possibly severe consequences for any of your ship's personnel assigned to the mission should you fail. In the event of failure, the target ship would be destroyed.
What happens after that would vary. They could be turned in to your faction for study (Starfleet) or as a trophy (KDF). They could be used for special infiltration missions (both factions). Or, and this would be KDF only, they could be kept and used by the player.
Keeping a catured ship for yourself would not be as simple as claim and fly. The ship would be severely damaged, it's interior and life support would not be optimized for your species, and most of the ship's computer systems would probably be locked down. Making it usable would require work and resources (not unlike the fleet starbase system).
It should be noted, that I would not make these ships anything special; in fact, many of them would likely be inferior to their player faction counterparts (it wouldn't do to have them replace conventional player ships). It should also be noted that many NPC ship models are not built to the same standards as player ship models; personally, I could live with that.
I'd also expand the PvP system to include a territory control sector; where the factions fight over the various systems, to receive various faction bonuses. Each system would consist of 1 or more planets/moons/stations; which require both space and ground victories to conquer.
Obviously, if it were just players, then these battles wouldn't be particularly epic; so they'd be more like warzones, with plenty of NPC mobs and fixed defenses.
Personally, I'd have each planet have a number of different ground warzones (a city, a military base, an industrial complex, etc); all of which must be under the control of the same faction before the planet stops being 'contested' and becomes 'claimed'.
since two of your three suggestions are about "real exploration" - I would suggest playing Spore - it's going to be way closer to what you seem to be looking for.
i would also fix the KDF and add the RSE like the OP said and also monster play borg and etc. so i agree with what the OP said and the next statement after that also piracy that would be a good idea as well like have players be able to do like trade and etc and fly freighters and so on and the chance to be attacked by pirates/orion or what ever they are.
i would actually add exploration back to the game because technically that is what star trek is all about exploration, to find new worlds, new civilizations, to bodly go where no one has gone before. And with the additions of fleet star bases, like for example if your ship gets seriously damaged, just like in the shows, you stay that way until you make it back to your starfleet base to make repairs.
Now in my opinion that is STAR TREK and is what STO should be more like.
1) I'd redo the game engine it was never meant to handle something like star trek e.g. you can't be in your bridge while your ship travels to it destination and it will likely never happen in this game.
2) instead of an exchange i would have a merchant faction where you could sell your wares to starfleet, KDF and other factions. you could get item either from your alts through an account bank or you can decide to do raids where you steal from stations, planets or other ships. The raids would make you notorious so you could be attacked by the faction that is control of that area. big jobs like raiding a starfleet base would have to be done as a team with other players.
3) I would deside on a war and stick to it! none of this we're at war with the KDF but we but aside our differances 99.9% of the time to work together for the common good so we don't really know why we are at war anymore but we are.
4) start the game off as a student in that faction's academy for the tutorials the jump x amount years later and your a commander and first officer and your captain is killed you take command, ship is destroyed but you can't be held reponsible for it and you saved the crew etc etc admiral quinn likes the cut of your jib and gives you a new command with a new ship etc etc but your not promoted to captain until later on in the game
Rewind the clock, and forget the Doff system, forget the Fleet Starbase system, forget the Shooter changes, and direct energy toward Exploration and Story content. Also, stop adding mish-mash content into sci-fly blender. And everyone must wear the same Starfleet uniform.

I'd replace exploration cluster "boxes" filled with generic sensor readings with exploration from the ship bridge, so that you have no real clue about where exactly your ship is located.
EDIT: And replace ship consoles with more specific ship components like hull plating, sensors, warp core, etc.
Fix all bugs. Merge the KDF with the rest of the game. Ditch PvP. Put the lockboxes back in the doff packs. Finish the storyline with a twenty four episode final season. Give the Foundry all of the tools it needs to make UGC and refocus the dev team to support it.
