This is pretty much a player guide to completing title quests. Its up to the players to work together to figure out the answers. Think of it as an egg hunt. Why would I ruin that for you?

I'm having our QA team look into everything that you've all mentioned. Please note that the Crimson Imperium questline isn't complete until the new instance becomes available in the next update/expansion. This is pretty much a precursor to a much bigger update that will be coming.

Personally I use both yahoo and gmail and I've had to wait up to 12 hours on both before I received an email. Most times its almost instant but there are some instances where I've had to wait quite awhile.

Technically with the new time guidelines people were thought they were 4 year accounts are actually 5 year accounts. People who are receiving the 5 year packs are most likely getting them from people who just didn't want their pack and therefore handed them their code to redeem. You can keep the 5 year one and request for the 4 year one that you missed out on ONLY IF that was the one that you were expecting to receive this year.
