May be you took too much damage and got overkilled? Like in Warsong if you have too many mobs on yourself and getting overkilled you are not teleported to the start point but getting teleported out of instance. Never met it anywhere other than warsong though.f:confused

Can't agree with the view that teams shouldn't be allowed. You have time to squad up with people you play with and fight back to back - that is what nations wars are about.
