Ok, just found out, to open you just need a squad of at least 4 people and all enter the maze. Then you squad leader gets option to start instance based on leader's level range. still don't know the set bonuses though.

Gotta make sure I log into right dude! I can see why there was no code, someone would probably find a way to glitch the code like pretty much every other code (I'm looking at you d.orb code!) to get a bunch of multiples.
