This is happening to me as well, on PC, mostly alliance and guild chat but it usually takes me a while to realize there's a problem and not people just being quiet. Usually I just relog an it resolves the problem but it's still a problem for me.
I had this problem right after Mod 13 launched. It turned out to be, at least for me, the Earthreader boots from Omu. As soon as I took them off my companion stopped disappearing.
I just upgraded mine to legendary without any problems. Check to see if you're wearing any gear from Omu that you got from hunts. If you are, try removing it. That solved my problem.
Edited I think I know what the problem was with my companion disappearing, it's the Earthtreader boots. I think the problem is whenever the blessed land feature kicks in. Don't know how or why but I switched from Earthtreaders to Bonespurs, the problem stopped and the tiger stopped disappearing. IDK if it was a fluke and…
It's the weapons that are bugged but I think it's not limited to the weapons. Even if you unequip and use a different set, the stuck in combat bug is still there, just not as severe. Happened to me in Dread Ring using my Chult set(Teak/Teak)
Companion is going from summoned to active at random moments. Not so bad if you're not in a fight but nothing you can do to re-summon if you're in combat.
I played archer until I hit 70 and then i switched to melee and having a lot more fun. I do keep an archer build on my second loadout for those rare group occasions where getting up close and personal is a little too squishy, but by and large melee is a lot more fun.
