1 and 3. I'm not sure why we really need to go beyond this. It wouldn't even be a problem if people got some really naggy warning windows when they get reported a bunch, either. But silence/banning needs to be a live human with accountability (so possibly volunteers, in a properly arranged system, but not the mob).
Taunts and, broadly speaking, almost all forms of aggro management in MMOs are stand-ins for the vastly harder to simulate 'get in the enemy's way to keep them from attacking your squishy healer'. Since NW has a threat mechanic and has thus chosen this shortcut, it needs to make it work.
