Bad idea to not allow companions in town. They get a good amount of xp from your invocation so you want them with you while you're trying to level them up.

It's a bug, usually related to changing appearance of your companion right after logging in, but hard to duplicate. I've had it happen several times on my CW; the second one disappears when you zone/change instance.
