I noticed the animation for non-epic mounts has been adjusted to be smoother for epic speed. Before, a green non-horse would move as if it were on crack, the animation accelerated with the mount speed. Maybe you can better compare when you switch speeds with the same mount selected?
I had the same issues, it can help if you change instances. I don't know about the fire, but for the lures (beasts & yeti) it happens when another player activates but doesn't finish it before you arrive (and drags the monsters away not to return). Maybe it takes some longer time to reset the trigger than you were waiting…

Happened to me, too, when PC was lvl70. Since you don't "need" the XP anymore, the campaign quests aren't highlighted to you, you must run around and explicitly ask any & every possible quest giver to pick up the quest chain again.
