This bug has been around for at least 2 or 3 months. Unbound mass life scrolls work, but it's hard to determine which one of the account bound versions don't work, and I have multiple stacks of those depending on where they come from.

You should be able to pick up worn tools from the original supply box behind the crafting table

Had a similar issue last week: applied a crafted Greater CA Armor kit to Curselord's Assault Bracers. No tooltip, and I can't tell if the buff is active.

I think this issue is related to the entire Underdark campaign, not just the Maze Engine quests. Had similar experiences with Dwarven King quest arc--first chapter.

Agree, happened in Fiery Pit yesterday and Pirates Skyhold today. Plus, the mob chases you back across the map. I respawned at the campfire, only to be immediately surrounded and killed again. And again. Couldn't leave the zone, escape/exit out
