You can do all quests even if you are way above them. I did the quests in Neverdeath when I was level 43 or something and it is a level 26-30 zone. I already found several items, bracers, helm, boots that all have no actual model and just change the way your chest armor looks. Bracer: Piercing Antagonist's Bracers (Level 44) Boots: Greaves of the Red Dawn (Level 54) Helm I don't know anymore because I no longer have it. It was a blue level 40ish Helm i believe.

I found the skill Daring Shout to be very helpful. It generates determination for every target hit, but not only in front of you but in a 20' radius around you. The worst thing is, you outlevel a dungeon/skirmish way to quickly. For example Lair of the Mad Dragon. It is a level 34 dungeon and you can't queue for it with level 36. Why can you queue when you're 4 level below recommendation but not when you're 2 level above? At least make it +-4 levels or something.

There is no maximum amount. The conversion from rough AD to normal AD is limited to 24k per day but you can get an "unlimited" amount via trading. Some people say they made up to or more than 1,000,000 AD per day by selling level 60 items.
