Stuck at Loading screen - Found the config option that causes it

For years I have had problems where I get stuck on the loading screen. This is the screen AFTER the Cryptic splash but before the character select. This problem started with STO and since the engine is so similar (if not the same), the same problems exist in NWO.
Here are the symptoms I used to get in STO.
1) The game would patch and it would take 10+ minutes to get past the loading screen. Possibly more or possibly never.
2) I usually had to end task to kill the game
3) Go into the load options and start in safe mode.
4) Change the settings back to what they were - usually the highest settings
5) Subsequent logins would work quickly.
- I eventually found out what was causing it within STO: lighting quality other than low.
Same problems exist in NWO but the graphic quality is significantly reduced when you have this setting and it is dreadful to look at. When I change lighting quality to High, the game gets stuck at loading.
This has been a problem for me for years. I have had multiple computers/laptops since then. The only thing that is unusual about my config is that I have a Nvidia Optimus based laptop (dual video cards).
Has anyone found a REASON why this is happening or figured out a way around it. I know there are tons of threads on it but I've never seen anyone identify WHAT is causing it or HOW to get around it.
