I started out with a stack of 8 lesser bilethorn enchants in my inventory. I opened my weapon enchant window and dragged the stack on there, which put one on the item. Then I hit the "refine" button.
I poured feeder enchants into it until it was ready to level, then used a greater mark of potency and a coalescent ward to upgrade it, with another lesser bilethorn enchantment as a catalyst. It upgraded successfully to a regular bilethorn enchantment.
I then closed the refine window and saw on the item that it still showed the lesser bilethorn enchantment in the socket, it did not reflect the upgrade. I had missed that after I socketed it I had not yet hit "enchant', so thinking that the display had just not updated to reflect the upgrade, I hit enchant.
When I hit enchant, it socketed another lesser bilethorn enchantment (pulling a different one from the stack in my inventory), but the regular bilethorn enchantment I had upgraded had completely vanished. It was not in my inventory nor on the weapon.
Net result is that this game bug ate my regular bilethorn enchant.
Please note this enchant destroying bug (the refine option on enchants should not be available on enchants that have not yet been confirmed) and please restore my missing regular bilethorn enchant.

Oh dear. A couple of us had reported variants of this bug while it was still on preview, but only using radiants and the like, and I think both of us only got as far as refining, not upgrading, so we lost our points (wiped when we committed the item we'd been refining to the chosen slot) but not an entire enchantment like that.
That's great. So they knew about it in test and didn't bother fixing it before release? This is a real simple fix. If the enchant button is enabled, disable the refine button.
I know I and at least one other person reported that bug on preview awhile ago. I even gave steps to recreate, although I was only using regular enchants. I did go all the way through the upgrade. It reverts back to the type of enchantment you originally dropped in the slot before refining/upgrading, and everything you used is gone.
It happens on companions too if you don't hit commit before starting to refine the stone. You have to make sure to click the 'Enchant' button on the item enchant screen or the 'Commit' button on the companion screen before starting to refine. I'm not sure why the option to refine is available on the slot if the item hasn't been fully slotted yet. That seems like the easy check to make before activating the option in the menu - not slotted/committed, grayed-out; slotted/committed, can be clicked.
