I've fallen in and I can't get out

I have found a location in the Sea of Moving Ice in which you can fall into a hole and there is no exit. If you go to coordinates 4409, 159 you can look down and see a hole. If you drop down into it (the first time it happened it was an accident) you land in the water with a bit of land next to you. There is no apparent way out expect to use the "defeat me" function.

    There is also a place in Mt. Hotenow (PS4) in the lava flow that will drop you under the map (out of map) and no way to get out except to defeat yourself. Found this on accident jumping around on a mount. Not sure the exact location but if you follow the lava (in the lava) you will find it. Pretty sure it's the same on pc and Xbone.

    I was able to recreate it several times, so it should be easy to find. But Alice, didn't to die to find here way out. :p

    Until now I got trapped in Mt Hotenow in the lava river and in two places in the Sea Of Moving Ice, one of them was inside of the ship wreck with the treasure chest behind the wooden bars. ^-^
