  That's no rumor, and they will be fixing the tooltip to make it clear that you get 25% of your AP every time you're off cooldown, which is 30 seconds.
  
  This is no different than how most "you have a chance to" boons and enchants actually work. It always happens, as soon as it's eligible to happen.

  I think it is about 40%. I'm not sure about the cool down, I've had it proc twice in a matter of seconds before (when used in combination with the DC sigil).
