Corrupt game character/weapon animation

My Devoted Cleric character seems to have a bug whereby all the animations from using weapons are not linked to the actual weapon. They all originate from the floor between his legs.
Changing weapon makes no difference.
I have also noticed this on a few other peoples characters I have encountered in the game, so it's not just mine.
I have attached an Xbox One Upload clip showing what I mean.
I have already tried a hard reset and re-installation of the game, but it made no difference.
Can you please look into this, and hopefully fix it and possibly find a solution to prevent it in future.
