Which are bugged? (GF)

So obviously certain powers glitch at the 4th rank. Does anyone know for certain which of all the GF powers suffer from such bugs and which still function properly at max rank? I know Desperate Survival is bugged (from personal experience) but if you're aware of any others then please share. Or if you know of another thread that contains such a list then share that too. I haven't been successful in finding the info I want on the forums so far.
