I've seen this a few times. Lots of mobs around, and for some reason one group of them has like a segment health bar instead of the 3 that all the other ones have. It's like they're part of a major HE, but there are no HEs to be found, and they're just kind out in the zone. Did somebody miss those when adjusting mob health?
Yes, someone likely messed them up. This issue exists since Module 3, some mobs are way tougher then others.

I just figured it's a feature, and encourages situational awareness.
In the Fiery Pit in particular, little mob groups will sometimes be normal, sometimes elite -- so when you're doing a kill X quest, it pays to give them a good inspection before you just jump in and start whaling on them.
That's called "elite npcs" in MMOs and it's a quite common thing. They usually give a lot more XP and sometimes unique loot. Here, the loot part is of course missing...
