as the tooltip says i should get 25% + 7.5% + 7.5% at rank 3 that means 40%, or it isn't working like that?

The 7.5% is the HP threshold where it activates (i.e. 15% at rank 1, 22.5% rank 2, 30% at rank 3), not the amount you heal. The amount you heal is a flat 15% before HD and incoming healing bonuses (and offhand).

Not bugged.

Too bad I am not a rogue and sometimes stack too much CD on my tough enemies - . -

Oh and of course I am basically instakilled if they have that perfectbloodtheft too.
