Phoera strange bug

I took a Mystic Phoera few days ago and noticed a bug (strange ) with him.
Sometimes he "dies" or "unsummon" (his picture disappears), back when the effect it no longer works. I did various tests, and the only way to make it back "unequip and equip Phoera again".
Can be this bug affects other companions with similar powers (like Angel or Skull).
