Quest banner changing to Anvil

Can anyone explain to why this is happening, and what can be done against it - and or prevented?

Prior, it showed a banner of Neverwinter enclave, and still does in the Campaign window.
Now, just the anvil.

Do I redo the starting map or something, and republish?

    It seems fine when it shows in my campaign now, in the list...but no banner when its just the quest. This is so weird. Is it a caching issue of somesort?
