"Icewind Dale: lairs completed" after finishing the Death Forge quest

Hello,
after today's patch did I enter the Death Forge as the first thing, found it working again, thank you, but at the end did it now pop up an achievement, saying I had completed the lairs in Icewind Dale.
This seems obviously wrong, since the Death Forge is in the Dread Ring.
What was even more surprising and got me stunned was the arcane coffer contained The Eye of Lathandar!! Yay!! I suppose I just got very lucky (and it has nothing to do with a bug).
Another curiousity is that while the Death Forge rewards one with a bundle does the Phantasmal Fortress, which I finished directly after the Death Forge, return the usual array of items and not a bundle. Is this intentional? I had hoped it would now return a bundle on every day and for each of these quests in the Dread Ring.

The quotation marks and the wording used in these messages seem a bit odd, too.
