Bugs that my pre-made group routinely encounters

These are just bugs I have experience with that happen a lot:
1) Cannot invite everyone I know. They cannot invite me. Fix: someone logs out to character select relog in and now invite options are available.
2) Chest unlootable while other people in DD have initiated an incomplete vote on a ground dropped loot.
3) Join instance such as skirmish with a 3 person pre-made and occasionally a single player disappears from the group, gets booted, and does not appear in the instance.
4) Cannot join instance because old CTA instance is open and will not close.
