I finally put my finger on what I hate about the controls of this game.
While leveling up my friend remarked, "Arrrgh why did I stop attacking?" And I said, "Dunno but it happens to me too". It took me all the way until level 60 to put my finger on how it is you stop attacking in this game (as a GF). I finally figured it out, and it annoyed me enough to this point that I thought the world should have to suffer through this post.
Here are the steps to recreate what I consider to be a bug, and if it isn't a bug it is surely some annoying engineering to new players:
Step 1) Hold down left mouse on enemy target and start attacking away.
Step 2) While still holding left mouse, tap right mouse. Your right mouse at-will goes off with both buttons pressed and your left mouse at-will stops going. (intuitive enough... I think this is the ideal behavior).
Step 3) Release right mouse while never having released left mouse at all.
Now your sitting there holding left mouse at-will down, but you are doing nothing! You stand there like a dummy while the mobs hit you, and all the while you are thinking, "Dude left mouse is down why ain't I doing anything?"
I think the fact that one mouse button is being held down should be respected. The ideal behavior is after Step 2 and on into step 3 the left mouse at-will starts going again.
All that said, now that I know... I can train my muscle memory to re-tap left mouse when I right mouse and I should avoid the problem, but I think you guys could do better.

Hmm? I just tap the left or right button to use whichever at-will I wanna cast. The exception, of course, being ones that you need to hold down the button for - like ray of frost.

It comes up naturally when the left mouse is an auto like Cleave and the right mouse is a gap closer like Threatening Rush. If you are cleaving with left mouse depressed and you hold it, and if you get pushed back you might naturally threatening rush by tapping the right mouse without releasing left mouse cleave. Having done so, the expectation is you are cleaving away because left mouse is still depressed and has never been released, but you arn't for reasons I outlined in my first post. You are doing nothing.
It seems obvious when I write out the steps about why cleave wouldn't be going with left mouse depressed, but its not obvious if you stumble on it the hard way. As my friend and I demonstrated, you can spend a fair bit of time pondering why you suddenly are not using your at-will even though the mouse is held down. It doesn't even seem ideal.
I'm kind of inclined to agree that it is a bug -- have you bug reported it online/in forum topic?
I know I am constantly holding down the left button (or the right) for my at-will and then tapping an encounter key, the encounter attack occurs and my left/right mouse button at-will continues after the encounter. Mouse ought to work the same.
I think it is WAI as some skills you can't hold the button down or the skill won't release. For instance, the HR split shot, you hold the button down and the attack doesn't release. you hold the button down only to charge the skill then release the button and skill goes off.
I've always known how this works. And I think it works the way it should. When releasing the right button, just retap the left. Is it really that difficult? I mean, it doesn't require much dexterity to do that at all.
I won't comment on lazy fingers, tho' (Yeah, I know: I just did) LOL
What annoys me the most is that if i get proned or otherwise CCed, even if i've been holding my shift-key constantly, my GF won't hold up the shield when the CC ends.
Since there is often no clear indication when you are free from CC (the purple bar shows only on some skills, and animations are often out of sync), I usually end up without a shield when I desperately need it.
