Every so often I get some old dialog boxes pop up with cancel/yes choice for the things I've done before. On logging off for example a message about deleting mail. I've lost a couple of attached (just bought on AH) items this way. Maybe it's my fault for not reading every dialog box (but after a time who does it really). Recently I've lost a greater mark and R7 enchant this way because when buying pres wards in the Zen Shop instead of 'are you sure you want to spend 100 zen?' I got 'are you sure you want to refine without wards' message. The only clue that sth was amiss was a sound of burnt refinement components. It's annoying to have to hit OK when already logged off to change character but when you start losing items only because the system is bugged is another matter. Anyone else with similar issues? I'm not really sure if it's about my internet connection, settings etc or a 'known issue' never addressed.
I'm having the same problem, it is not a permanent issue, it occurs from time to time, but when it does, it's very costly and annoying. It appears without any noticeable reason or cause, there is no apparent lag accompanying it or anything like that. Those recurrent dialog boxes aren't even limited to poping up one extra time, often I have to cancel several such popups one after another. I can't see why such issue occurs if the action to which a popup refers has already been concluded some time ago.
If you can remember what dialogs you triggered just before the first wrong dialog happened, that'd be really helpful to figuring out the problem. Usually, it'd be something like clicking a button twice quickly (where the first click causes the confirmation dialog) but the 2nd click lands before the dialog can appear. Or if you clear the first dialog in an unexpected way (are you pressing Escape, or clicking Cancel, or toggling Alt?).
Any specifics would be really helpful in figuring out the fix, since right now it's "some dialog in the game is broken sometimes" which doesn't really help, alas.

1.1 before logging out/changing character I'm always checking my mail and deleting redundant mail
1.2 every time you delete a mail you get a confirmation box so I click OK
1.3 log out/change character
1.4 again a dialog box that I will be logged out in 15 sec so click log out
1.5 when the character menu appears I get another box on deleting mail
1.6 so accidentally (unaware of the consequences) I click OK
1.7 next time I log in the last msg (with attachments) is nowhere to be found
2.1 in a refining menus trying to upgrade R7 to R8
2.2 all me Pres Wards are burnt so I need a few more
2.3 I open ZEN store and click buy on Pres Wards
2.4 then a box appears so I click OK
2.5 suddenly I hear an ominous sound so I check my refining box and lo
2.6 both reagents are gone (R7 enchant and greater mark)
2.7 no Pres Wards in my inventory
2.8 so devastated I log out
2.9 in the character selection screen I get a dialog box if I'm sure i want to spend my ZEN on Pres Wards
So that's it. I submitted a ticket on the refining situation but not hoping to get the lost items back I've already spent ~230k AD on another R7 and greater mark. This is just annoying. True, I'm jumping between my toons quite often to finish dailies, fill in the gaps in the guild runs and just to try to keep all of them up to date so I don't pay too much attention to all of the system dialog boxes. It's more like a mechanical OK/Continue/Go Ahead as I just like to think I know what I'm doing at any given moment. Alas, every so often I'm wrong. I'll try to get some screenshots next time this occurs.
The trick will be to catch the FIRST wrong dialog box, then remember what confirmation dialog box you closed just before that (and HOW you closed it). Thanks for your help tracking this down!
