Fatal Error: Direct 3D Driver returned error code

After getting the patch last week, i have been getting this annoying msg...at first it does not happen often, but after yesterday this "thing" have been appearing as and when it like it and it came to find me quite often. Don't mind please fixed this, i had send in 2 tickets about this issues but so far still no reply or anything, below is an attachment of the 3D error code I had and I do not have other information other then this.
