The Forgotten Vault - unable to progress

Trying to complete the "Artifact Recovery" quest I've run into 2 bugs in the Forgotten Vault zone.
1) I had progressed to the 3rd platform. I killed the first wave of mobs but died to the second wave which includes the Shocktroop Devil. I ran back to find the first wave there to be killed again - after killing these the 2nd wave wasn't triggered again and so I wasn't able to interact with the skull to progress to the next platform.
2) Was fighting the Bone Golem on the 5th platform on my Ranger Hunter. The Golem was near a walled edge of the platform when I used Seismic Shot which not only pushed him up against the wall but through it. I then found the Bone Golem was now out where the 6th platform would appear if I could interact with the skull, but I was unable to because the golem was still alive and now out of range to be able to kill it.
