First strike question

Has anyone tested this? I'm wondering how often it triggers. Is it basically once per combat? Has anyone ever used it with any success?

    It triggers allways as soon as u enter the combat, the first and only hit u do it will be with 15% more dmg on lvl3 means it will not be used till u exit and enter combat again
