It's like when you created your character. You can't change race or gender, but you can change your appearance, including tattooes, skin color, hair, etc.
Unfortunately, though, it's bugged right now. You can't see your character in the editing area, so you can't see any changes you're making.
Within race, class and gender you can change how you look, hair, skin colour, tattoos everything. It is broken now. You cannot see your character. You can use it however, I did, but you go more or less into the blind. If you want to change some minor things, like adjusting some sliders, you can see the result in the blue marked window. If you want to do a complete make-over, wait for the fix.
I think it is pretty expensive for just cosmetics. Sometimes you just want to change some details, different hair, another colour or no or another tattoo. In Startrek Online it is 'free', it costs just game money, that would mean gold in this game. I love it and use it a lot. Sometimes you get bored with the looks of a character and also, the character doesn't always look as you want it in the game. Skin colour looks too light, legs do look too long or short when running. Minor details that can easily adjusted in STO, but here they cost Zen. They should sell you two at least or half the price.

You can buy Zen with AD, and you can buy AD with Gold.
Thus, you can use the character remodler by spending in-game currency.

Basically true, but STO is kind of built around people being able to change their look easily & frequently. Like, every 10 levels you get a free costume change so you can update your uniform with new rank symbols (or whatever else you want). It costs credits the rest of the time,the STO equivalent of gold, but it's cheap - if it was in NW, I'd say that changes would cost anywhere from a few silver to a gold, max. (the cost depends on how much you change - switch around some costume pieces, change a color: cheaper; throw around all your body & face sliders? That'll be more expensive.)
edit: like, for the recent summer holiday event in STO, I made up a couple "beach" costumes for each of my characters who participated. There was an appearance change NPC standing right at the beam-in spot for anyone who wanted to do it.
edit: Of course, costumes in STO are just that - only a little of your actual gear shows on the character (weapon, class skill kit, etc), but you can turn off most of those visuals so that it's just your uniform showing, like in the TV shows.
