Voice Chat not working

A friend of mine is having trouble with voice chat.
He is using a USB headset mic, but whenever he is in a party the option to voice chat isn't even there.
He has tried a normal mic also and still nothing.
The test microphone option also does not work.
Anyone have any ideas on what could cause this problem?

    I'm getting the same issue as the issue appeared after the recent patch. Can't use the test microphone option as there is no "Start Recording" button displayed.

    Worked fine pre-patch. Not now.
    Current Issues:


    1. No option to have Hero of the North title on forum because option does not appear to exist in settings.

    If these issues can be solved and I'm not online, then I thank you.

    Ditto.. we were playing fine, three of us voice chatting without issue. Third friend joined but couldn't hear us or chat. So we all left the group and rejoined.. Now no one can use voice chat.
