Couldn't get map names from map manager

Third time getting this "[Admin] Couldn't get map names from map manager".
Yet another DD chest I cannot get.
First & Third instances that this occurred was during a pre-clear for dd event. Once dd event became active I relogged while someone else stayed at fire camp. Loaded up The Chasm map and clicked Return to Instance ... booted from team and error message.
Second time this occurred was during dd event but was not a pre-clear. It was during spider lair and I relogged, exited dungeon ... clicked return to instance ... booted out of team.
This has happened 3 times to me within about a one month time frame. During this same time frame it has happened to at least 2 other people who were in my team.
Getting this bug is ruining our fun and its just so frustrating. Now the party has to attempt a 4-man epic boss fight which gives everyone un-necessary stress. Twice the remaining party members were unable to complete the epic dungeon which is a complete and utter waste of their time ... no reward for our hard work.
