Item disappeared..please help me with this problem.

I went to Spider DD Tier2 and i got 'Avatar Scale Amor' from delve box but when i came back to town the item was not in my inventory. What is this? I ran up for spider for days straightly and i finally got it.. and it was disappeared..
so f stressful .. please help me with this .. omg..
