Instances, what is it?

I have tried, tested, failed & learnt everything i know abt this game. but instances are still a blur to me, i know i can change instance within a map to join a friend or just to farm creeps again.
is it possible to leave a dungeon & return to it? without having to reset it?
Is it possible to reform team if someone quits at final boss in dungeon? (wastes everyones time coz party of 4 in tier 2 dung is near impossible)

Throw Basic info abt instances too, if u may.
