Dungeon Finder Queue/Party Issues

Didn't see this mentioned in the last 3 pages, but a lot of the times, when you queue up for a dungeon or skirmish and it accepts, and the party is formed. Some people will not be put into the dungeon, nor can they leave the party. Also related is instead of putting a healer/tank/DPS classes together, it'll sometimes just throw in all 5 Rogues, which is fine at level 20, but not at 60 doing epics
