Died just after finishing a quest.
Initially there were options on-screen, Call for Help, Release - but unable to click on them (plus a rapid count -down timer that didn't give much time to try and figure what my options were)
See screen shot (edit - doesn't seem to post screen shot even though the IMG tags look okay.

While Dead I could see 2 options - Call for Help (grayed out) and Release but could not click on Release
Left or Right mouse button only gave a message that I couldn't use that power.
Tried every other key press.
Tried using ALT and while I could move the cursor a small amount Release Button, it didn't work.
Question - What does the 5h-1 under Call for Help button mean?
Question - What does the 5h-2 under Release button mean?
While Dead, I was able to change location. Is this normal behavior?
Checked under key binding to see if there was a keypress for Release but found nothing.
Solution was found accidentally trying to find a solution:
Options: Rearrange Hud
Once I clicked on that, I could interact with the Release Button and respawn the character
