Mythral set missing

Hello. On my wizard I appear to be missing my neck and waist of the set. I have the artie, but I can't locate the rest. I havn't used the forms in awhile, so forgive me for not finding all the info to set up a proper ticket. Do you need the email associated with the Xbox account? And where do I submit tickets now?
