Mask of the Fallen Disappeared - can't get it back from NPC

Mask of the fallen has disappeared from my character slot and inventory. Trying to get another mask from the team leader NPC also fails. NPC goes through discussion but mask does not appear in inventory. Unable to participate in Masquerade of Liars Event.
