Area dungeon death bug

Sometimes when I finish Phantasmal Fortress in dread ring or one of the glyphworks in Vallenhas. I come out of the instance dead and need to release to the nearest campfire. This has been happening for about 2 months now.

Are you wearing any gear that lowers your Hit Points? There were similar incidents of people dying when changing maps because of a piece of gear from Vallenhas. I think the Barbed Cultist Greaves but not sure.
