If I remember correctly, the combined rating was intended to compensate for the loss of stats from the increase in overall item level. Therefore, it would be logical to assume that items with the same item level should have the same combined rating.

Mount collars are meant to have very little combined rating as they give % bonuses. % bonuses tend to be more valuable so the devs didn't want to give combined ratings.
Yes... for collars, two of them have combined rating == item level and the other three have combined rating == 50% of item level. So that is normal.
For the comment about insignia... I'm not sure that's right. I think for something like insignia of prosperity, the gold and glory gain do count as stats and the remaining hit point bonus is less than if it were the only bonus.
I hadn't really noticed different combined ratings (as a proportion of item level) with artifacts... but you certainly get that with other eqipment, like rings. It's all part of balancing the stats... the less combined rating there is the more focussed the gear is on the stats it gives and the bigger effect it has on the percentages for those stats. So for some builds, gear with a low combined rating is desirable.

No. There is a formula to calculate item level and I provided that. For the same item level AND with the same amount of regular stat, you will get the same combined rating. In your examples, the regular stat numbers are not the same among your equipment.

Some time ago, there was a patch note indicating that the collar types that don't affect combat (I have gold gain and movement speed, IIRC) got the extra IL where the other three types remained the same.

For the collar that does not give combat related improvement, it has more combined rating to match the item level.
In the above, +400 combined rating = 400 item level. 4% damage is another 400 item level. Hence, it has 800 item level.
In the above, 8% rAD does not give combat related improvement, it does not give any item level. In order to have 800 item level, this insignia has 800 combat rating to reach 800 item level.

+400 combined rating means +400 to all 15 stats.
They can all have different combined rating including no combined rating.
Using above as an example.
Item level = ((combined rating x 15) + sum (all regular stat)) / 15
((1020 x 15) + (900 + 900 + 900)) / 15 = 1200 item level
Since gold gain is not a real 'action' stat, it does not contribute to item level. Hence, the combine rating for that insignia is higher than others.
