HELP! Exploration Achievement: "The Story Of It All"

"Collect all 6 entries concerning Flora, Dwarven History and the Fallen Lovers"

Thats all it says, no Area or any COUNT of how many already obtained or clues where they might be.. anyone able to complete this or give tips?

    There are 2 entries for the 3 starting areas. 1 in each zone is basically given for free upon entering the area - Whetstone Thicket, Quellwater Tarn and Fallen Lovers pt1. The extra Thicket one is obtained near the scrying stone and Wolf miniboss. The one for the Tarn is inside the large Dwarven structure that water flows out of. The Fallen Lovers pt2 is along the wall near the Dragonsight artifact where the 2 large dragon skeletons are.
