TIC Teleporting Bug... Again

Once again here to report that the TIC teleporting bug has not been fixed. I've just left a radq where we teleported between the Abyss and the Hall of Chains after queuing to enter the Hall. Once we all managed to get free, we were stuck in the 1st boss arena without the first boss. Killing ourselves and starting again just results in the bug again as the instance was broken.
It's difficult to explain how to reproduce - there's no obvious cause from the player side, but this is the exact same bug that was supposed to be fixed in a previous patch, and I have previously reported after said patch.
