The Upper Vault - quest/questgiver issue

I was playing with a friend in the Whispering Caverns and we were on the quest The Upper Vault. When we went to the actual vault - where you enter the lower vault, then go through another portal into the upper vault - I was able to get into it, but my friend got bugged outside and could not continue the quest or complete it. We attempted to fix it by them removing the quest from their quest log and reaccepting it, but the quest itself completely disappeared afterwards, along with the questgiver. Running around, we discovered the questgiver was somewhere else on the map, but still would not give the quest anymore, making my friend unable to progress to level 20 with me.
