Occasional Insta Death upon entering or leaving Vallenhas and Avernus weekly instances

I had it happen last week on my Barbarian upon exiting the Village Glyphworks...had it happen again on exit with Village Glyphworks this week too. Had a second insta death upon entry to The Fall instance. Hadn't seen these posted anywhere so figured I probably should.

    I've heard about something happening like this with Barbed Cultist Greaves.
