Forsaken Lockbox bugging out

Anyone having trouble with the forsaken lockbox? I've had multiple times where I have opened it up with my daily vip key and have it proc mount and or companion upgrade pack but then to look in my inventory to not see it in there even though it popped in the window on the right that I received it. I have gone to just showing unopened items only due to it being a pack that I would need to open in order to get the tokens. Just wondering if I'm not the only case this has happened to. Is this a bug or something I would have to send a ticket into if its just my case.
