PS4, paranoid delusion comp

Paranoid Delusion is doing something crazy when summoned, when it stops mirroring in combat instead of turning into the purple wraith looking animation it turns into a large pale woman and the lower half of her body has like long almost rectangular masses stretching out sideways.
The issue occurs everytime it stops mirroring the owner in combat.
Does it no matter what map I’m on.
