Relentless Slash/ very large monsters

For the Barbarian skill Relentless Slash, the description states that you perform a twofold attack to in a 240 degree arc to enemies in melee range. It does this well for most situations. However, for very large critters, mostly boss mobs but size seems to be the actual issue, this attack only strikes once, even at the closest range to the Mob as possible by object obstruction in the game. The first strike appears as normal, but the follow-up strike will often position the "spectre" in the middle of the mob, thus not striking. Battle logs confirm this. A great example for this is the Boreworm in LoMM. Is this mechanic working as intended for these types of very large creatures? If it is, the tooltip needs to be slightly adjusted. TY.
